IF YOU DON'T SEE A CLASS at a day/time that works for your 
child, let us know and we can help you find one. Note-age 
is only a guideline and NOT always definitive. Schedule is subject 
to change/revision.

PLEASE PRE-REGISTER FOR A CLASS(S) 
but if not sure due to child's young age, etc.,
you do Not have to make payment until trial 
class is taken but before student returns to the 
next class. This also allows to know you are 
coming and reserve your space in class.

CLASSES WITH 3 STUDENTS or LESS  -
WE RESERVE THE RIGHT TO CANCEL 
THE CLASS OR TEACH A SHORTER LENGTH CLASS FOR 
THAT DAY, UNTIL ENROLLMENT INCREASES.

**CLASS CHANGES-Transfer or Drop: If you wish to change to 
another class (and it is not being done on the day of the initial registration), 
you will need to email the Office and they will change or discontinue for you.
 ALL DROPPED CLASSES MUST be sent to the Office on the last day of 
month to avoid tuition DUE for next month.

TUITION & POLICY INFORMATION: Note-- 

NOTE: When registering into our accounting system, 
you have agreed to accept our company policy's,
 to make your payment after the trial class(s) and 
before the next class(s) taken.

**TUITION IS DUE by the 10TH OF EACH MONTH. 
A $15 LATE PAY FEE can occur if payment is not posted 
by the 10th. 

PRE-REGISTRATION AND PAYMENT Guarantees your 
space in class.BEFORE COMING TO A TRIAL CLASS...
Please register on line and select the class you would like to try - 
while NO prepayment is required before trial class, if you need the
class day/time, then your child's spot in the class may
not be guaranteed until registration payment is made.  If you 
show an Incorrect tuition amount after Registering, please 
do Not make payment, notify us asap by email and we 
will correct and send back corrected account.  If you receive 
an email stating "there is an issue with your online enrollment"  -- 
Please DISREGARD...no need to contact us as it a Default message.   
We will correct your account and email you within a few days. 
Tuition is the same each month whether 3,4 or 5 lessons. If less than
4, a student can take a makeup class-age/level appropriate. Monthly 
tuition is based on total tuition amount for the school year Sept-June,
divided by 10 months.

SIBLING/PARENT DISCOUNT: $10 per child/parent.Note - 
If using Boys Scholarship, can not also use sibling/parent discount.

  BOYS SCHOLARSHIP:  To encourage our guys into 
the performing arts, they are automatically placed on scholarship 
and can take more classes for no extra charge!  
Example. If a boy takes 2 classes per week, he will 
only pay tuition at the '1 class per week rate'. If takes 
3 classes per week will pay tuition at the '2 classes 
per week rate', and etc. If a boy only takes one class 
per week, he can use Sibling discount if has a sibling 
in program. Boys scholarship starts at 3 classes per 
week for Company division. Note-Sorry, NOT able to offer 
Boys scholarship in Ballroom program-DO offer 
Sibling/Parent Discount.

**ANNUAL STUDENT REGISTRATION / INSURANCE FEE:  
**1 child=$30, 2nd Child-$20. $50 Family Maximum

This amount is paid once a year by ALL NEW AND RETURNING 
students at the time of Registration.  
It is valid September 1, 2011 thru August 31. 2012. 
New Summer students that already paid $10, 
will only pay balance due. Note: Registration/Insurance fee 
is nonrefundable.

STUDENT DIVISION &  
BALLROOM TUITION:  Total monthly tuition
$  56  = 1 class per week per month
$ 100 =  2 classes per week per month
$ 140 =  3 classes per week per month
$ 170 =  4 classes per week "          "

COMPANY Team DIVISION TUITION:
$ 105 = 2 classes/rehearsal total each week per month
$ 140 = 3 classes/rehearsal total   "         "       "       "

$ 197 = 5 classes/rehearsal total  "         "       "       " 
$ 217 = 6 classes/rehearsal total  "         "        "      "
$ 235 = 7 classes/rehearsal total "         "        "      "

$ 275 =  Unlimited Classes/Rehearsals per week per month

$75 = BOYS Company team tuition-2 class/rehearsal per week 
$65 = 1 tech/reh class combined per week for any Company dancer

Odyssey, Company Div 2 and Apprentice - NO extra charge


JAZZERCISE / CLUB SALSA / NIA -- pay teacher directly-RANGE's $10-$15 PER CLASS-cash / check Only. Will not register and will pay teacher.
Jazzercise can purchase more classes at one time to save $.


*REFUND POLICY:   THERE ARE NO REFUNDS/CREDITS 
ONCE REGISTRATION HAS BEEN ACCEPTED unless a MD 
verification is received. Student can do makeup classes if they 
to miss the class they are registered 
into to. No need to contact us unless student gone for 2 plus weeks.
Can attend a makeup class for age and level-no need to get ok prior
to attending. 

DROP a class(s) - MUST email Office before the end of the current 
month to avoid a tuition charge for the next month. 
Please list the reason to dropping. No pro-ration for mid month
drops.

TRANSFER: Please Email Office with your change class(s) information.
Can Add new class but can Not drop existing class.


IF OUR ONLINE ACCOUNTING SYSTEM DOES NOT 
CALCULATE YOUR TUITION CORRECTLY - Please check 
back in a few days and should be corrected or can email the Office. 

HOW TO MAKE A PAYMENT:   

*CASH  PAYMENTS: Must be presented at Office and  
given receipt to verify payment. 

*CHECKS- please have the student's name 
written in the memo area and put in an envelope at Office 
Tuition Slot or can send by mail to: 2956 Treat Blvd., Ste I, 
Concord, 94518. Returned checks will post a $25 
Return Check Fee.

CREDIT CARD/DEBIT CARD PAYMENTS:   We offer online 
card  payments - VISA OR MASTERCARD only. You can choose
to have us do autodebit each month between the 3-6th of month
or you may self process by the 10th to avoid late pay fee-$10.

MAKE-UP POLICY  Make-up classes for any 
missed classes must be made within 30 days. No need to contact Office. 
Please choose a class of the same age and level of your child. 
All teachers will accept makeup students. If a student needs to be
gone from their class(s) more than 1 week, please email Office so 
when can alert their teacher(s).

LIABILITY RELEASE: All students and parents must 
disclose any preexisting medical- physical condition 
to the studio before entering into lesson agreement. 
DCPAC has the right to use discretion upon accepting 
any student. DCPAC releases itself and owners to any 
injury that could occur upon entering a dance lesson.
